Capital Expenditure Authorisations

I am trying to set up a form for Capital Expenditure Authorisation at my work place.

Can anyone send me an example so that I will know what to include ?

Well you aint had any replys so I'll have a guess if you like!

1) purchase price/supplier/requisition number/stuff usually appearing on PO etc (oh and serial number)
2)Reason for purchase.
3)Details of asset for register /expected life/residual values etc
4) Authorisation/signatures etc.

I think some companies even have a photo taken of it!
