Witness Statement/Workplace Evidence

Hi everyone,

I am in the processing of completing unit 22 (Heath and Safety etc). I have been told that what I've done is fine, but I need to submit a witness statement.

I have a blank form, but I am unsure what to fill in??? Can anyone tell me what goes in the range column and where is the witness supposed to sign the form??

Any help - much appreciated.

    I would ask your tutor if I was you as they are offically are the witness.

    Also your employer may help as well

    Hi

    I have a look at the one I did, and you have to get someone to sign it off that it is a true record of what happens at your place of work or scenario.

    My boss just put - I certify that this a true record and then signed and dated it and she signed it off on the front page of the project.

    Thanks for the advice. I have gone with getting one of my manager's to sign off the bits I needed. I have submitted these, so hopefully that's it.
