STUDENT RECORD - CONFUSED!?!?!?

I have (hopefully!) just completed my projec and am trying to fill in the student record for the portfolio but am having trouble knowing what to put under 'Evidence' - do I just put the project/project name?

    We were told to number each paragraph in the project then cross reference the numbers with the student record, this way you would/should cover all criteria required for the unit,adding anything that was needed to complete it. Hope this helps
