Unit 22 - Health and Safety help needed

Wondered if anyone can help me. I've been given a set of question to do for the Unit 22 coursework and one of the things I have to do is complete a health and safety risk assessment (Element 22.1 question 4). According to the question sheet, a risk assessment form is attached. Said form is not attached. Is anyone else that's doing the same work know where I can find it? Any help is much appreciated.

    I've got different questions to you in my H&S - 22.1 gives questions A-G, with D being:

    'Promptly and accurately report any hazards that you are not allowed to deal with to the relevant person and warn other people who may be affected'.

    None of the other questions mention risk assessment either.

    I'm wondering if you have the wrong one... or have I?

    The coursework can be set in different ways so our different questions simply reflect that. Neither of us has the wrong work.
