Witness Statements

I have had a trawl through the old threads and my student record but can't find the answer to my question:001_unsure:
I am working on the Working with computers unit, which is all pretty inane stuff since I've worked for 13 years so I am going to get a lot of the evidence as witness statements signed by my manager.
Is there a set format for these or do you just type a list of things you need to show you can do and get them to sign the bottom?
Thanks

Hi
Our tutor wanted it to be set out in a particular way so gave it to us and we had to copy it on the company paper and get it signed, so it might be worth you asking yours what they prefer.
