Is there an example 'authenticity statement' for unit 10, as I'm not sure what wording to use for mine?

I am also unsure what should go in here, can anyone let us know a brief outline of what it should say maybee? Should it just say your employer has read your report and verfies that it is true, or should they comment on the content of the report and state any changes that have been made or reasons for not making changes?
