I need to add a new principal activity to a client's limited company.

I know it will have to be minuted, but how do I inform Co House?

Can this be done anytime, is there a form or on PROOF, or is it done when the annual return is submitted.

No need to inform Companies House now.

Just add a SIC code when you do the annual return.
