How have you guys gone about making DD mandate forms for clients?
Is it a pretty much a simple process? Any good links out there?

Do you mean to collect dd from clients? If so then your collection provider will provide you with a copy.
I use eazipay for collections. All fine with no probs.
I previously used standing orders but it can be awkward if you need to make changes.Regards,

That was exactly our experience, SOs were fine until fees changed or the VAT rate changed, then it became a pain for having new mandates signed... etc.
We use London and Zurich for our DDs and, as Ian says, whoever you use should provide you with a form. L&Z were quite happy to put our logo on ours too.0
