Fixed Asset Acquisition forms

Hi Everyone
I am trying to tidy up my companies fixed asset register becuase it is a bit of a mess. I was considering creating a fixed asset acquisition form for everytime a new asset is purchased and i was wondering if anyone had a template or had any pointers of what i need to include on this form?
Any help would be much appreciated

    My adive is to use excel... and you need...

    Date
    Who it was purchased from
    Asset description
    Net
    Vat
    Gross
    how it was paid... i.e bank, c/crd, finance etc.
