Contractors Statements

Could anyone please tell me what needs to be included on the statement which Contractors are meant to issue to subbies in place of the vouchers?

The only thing i can find on the lovely HMRC website is a paragraph saying 'they can be in any format but must include certain information' :? helpful as ever...

    Ours include:

    Month ending

    Our UTR

    Their UTR

    The total Gross payments for the month (excluding VAT)

    Amount deducted as tax

    Total paid

    Verification reference for anyone that has had to be verified

    Plus all the usual info - addresses etc.

    And don't forget to add the contractor's PAYE "employer's" reference.
