VAT and the FRS

I am about to register my husbands business for VAT though the Flat Rate Scheme, which makes sense for us as we have very little input tax.
The question I have is how to go about registering. Would I register for VAT in the normal and then register for the FRS separately or do you just register for the FRS?

There is a separate flat rate scheme application form, you can send it in with the same dates as the VAT registration form.0
